TB skin readings are $30 (readings take place 48-72 hours after initial placement).

Family Medicine
Our patients depend on us to provide this high quality and affordable services. We offer a wide variety of services that are fully personalized and administered by our Nurse Practitioner who specializes in Family Medicine.

Days & Hours of Operation

Connection Medical Clinic is a volunteer-based medical ministry dedicated to serving our rural community. As a Christ-centered nonprofit, I operate on a charitable basis, providing care through the generosity of my time to patients who share a commitment to our mission.
I am in the office from 10 am to 3 pm by appointment only one Saturday morning/afternoon a month. I will additionally offer one Sunday evening a month from 6 pm to 9 pm to provide telehealth visits as needed (remote electronic visits only during Sunday hours).

As the founder and provider, I also work full-time in another position outside the clinic. Despite my professional commitments, I remain dedicated to ensuring that Connection Medical Clinic continues to offer compassionate, high-quality care to those in need.

Masks are required to be worn prior to entering the clinic per provider discretion for all sick visits.
Help protect our community and healthcare provider by wearing a mask prior to entering the clinic when you are ill.
Performing hand hygiene when entering and leaving the facility is essential in reducing the spread of illness.
A mask will be provided if you do not have a mask available.

About Me
Dr. Desireé Samples, DNP, RN, FNP-BC originally hails from Northwest Illinois and has resided in Northwest Indiana since late 2015. She is a dedicated Board-Certified Family Nurse Practitioner with more than 14 years of experience as a nurse practitioner. She prides herself on bringing a wealth of knowledge and compassionate care to her practice.
She is a devoted wife and mother of three children to Eric (15), Addelin (13), and Colten (10). Her family bonds over shared interests including hiking, games, and exploring new destinations. Additionally, Desireé has a passion for rescuing animals, she lovingly cares for her Cockapoo Winnie, and rescue cat Bruno.

Over the course of two decades as a nurse, Desireé has provided comprehensive care across all age groups. Her experience spans diverse healthcare settings, including pediatric intensive care, transplant intensive care, retail health, psychiatric services, hospice and palliative care, and private practice.​

Desireé's vision for the Connection Medical Clinic took shape over 6 years ago when she identified a pressing community need for healthcare that marries Christ-centered values with a commitment to excellence and affordability. This need encompasses individuals with or without insurance coverage.
